Who we are
The Continuing Care Team (CCT) is dedicated to supporting adults who are over 18 years of age affected by severe mental illness, their families, and carers. We operate with a recovery-oriented approach that emphasises consumer choice, resilience, and social inclusion.
What we do
CCT delivers assertive, recovery-oriented assessments and clinical interventions to minimise the impact of mental illness. We facilitate access to a range of clinical and non-clinical services, collaborate with consumers to develop personal support systems, and engage with Non-Government Organisations (NGOs), primary care, and other providers for early intervention and support.
Who can access this service?
Our services are available to individuals over 18 years of age most severely affected by mental illness living in the Gold Coast Health catchment area.

Current patients of Gold Coast Health’s mental health services can be referred by their treating teams into this service. New referrals into Gold Coast Health’s mental health services are through 1300 MH CALL on 1300 64 22 55. When you call this number, you will speak to a health professional who will ask you some questions to ensure we find the right service for you.
