From Monday 7 August, all services are operational at Southport Health Precinct; Level 7, 16-30 High Street, Southport.
The Adult Community Mental Health Continuing Care team provide assessment, case management and treatment for 18-65 year olds who live with an enduring mental health condition within the Northern Gold Coast Area. Case Managers work with consumers, carers and health providers to coordinate a holistic treatment plan aimed at promoting healthier lifestyles and environments for individuals, families and communities.
Palm Beach Adult Community Mental Health Continuing Care team is a skilled multidisciplinary group of professionals providing assessment, case management and treatment for consumers from the age of 18 to and in some cases over 65.
