Who we are
The Transitional Recovery Service (TRS) provides psycho-social support through the Intensive Residential Treatment Support (IRTS) program and the Capstone program. Our focus is on reducing prolonged hospital admissions and supporting early discharge into a brief supported residential setting.
What we do
The Intensive Residential Treatment Support (IRTS) program offers short-term residential support (seven to 14 days) to assist consumers with their psychosocial recovery goals. We provide early intervention to prevent hospital admissions as well as facilitating a smooth transition from inpatient care to community living. Our programs include individual support as well as clinical and non-clinical group support which promotes continuity of care and skill building. Participants in this program work collaboratively with clinicians from Queensland Health as well as recovery support workers from the Richmond Fellowship Queensland (RFQ) to identify and work on their mental health recovery goals as well as engage in activities to improve their understanding on their recovery journey.
The Capstone program is a psychosocial recovery program designed to promote mental health recovery through a combination of one-to-one support with our RFQ partners as well as clinical and non-clinical groups in the community. Eligible participants can receive up to four weeks one-to-one support with a RFQ recovery worker who can see participants at home or in the community. Eligible individuals can also access our groups in the community for three months once accepted into the program.
Who can access this service?
The IRTS service can be accessed by eligible individuals aged 18 years of age and older who are currently in-patients of acute mental health wards at Gold Coast University Hospital or Robina Hospital. Referrals are completed by the inpatient treating teams. Referrals can also be received by community mental health case managers who are providing support to individuals in the community.
The Capstone program is available to eligible individuals from mental health inpatient wards at both Gold Coast University Hospital and Robina Hospital as well as individuals who are currently linked with community mental health teams in the Gold Coast area.
How to access this service
Referrals for both IRTS and Capstone need to be made by clinicians within Gold Coast Health. The referral forms can be accessed through the Queensland Health Intranet.
Our locations
The IRTS program is facilitated at two residential homes located in Robina.
The Capstone program is located at the Gold Coast RFQ Hub at 10 Mick Veivers Way, Southport.
