Chronic Disease and Post-Acute Programs

Our programs offer community-based services to provide specialised care, education, exercise and support to help you manage your health conditions.

Chronic Kidney Disease Service

About this service

Many people are unaware that they have chronic kidney disease because there are often very few obvious symptoms until the kidneys are really struggling. However early kidney disease can be picked up in a simple routine blood and urine test (that your GP might have ordered for a different reason). We work with you and your GP to:

  • help slow the progression of kidney disease
  • prevent any potential complications
  • work alongside other health teams to help manage other conditions that may affect or impact the health of your kidneys

We provide education, monitoring and support during and in between appointments as required. We also run a support group every two months with guest speakers which you are welcome to attend. 

Can the service help me?

Yes, if you have:

  • Chronic Kidney Disease confirmed by your GP
    • Usually a blood test that shows kidney function is less than 60% of 'normal'

This service requires a referral from your GP or another health specialist (for example your Diabetes Educator) and is available at Helensvale Community Health Centre and Robina Health Precinct.

Diabetes Service

About this service

Our diabetes service is designed to help people with diabetes learn ways to keep blood glucose at an ideal level. 

We offer an individual assessment with a Credentialed Diabetes Educator, group education, and ongoing support.

Despite best intentions, it can be hard for some people to get diabetes under control.

Our Diabetes Educators will work with you (and your GP) to help you avoid the possible complication of diabetes.

We provide monitoring (including continuous blood glucose monitoring) and you can also see our other health professionals who specialise in Diabetes care such as a Dietitian, Exercise Physiologist, Podiatrist or Pharmacist.

Can the service help me?

Yes, if you have:

  • Impaired glucose tolerance or pre-diabetes
  • A diagnosis of type 1 or type 2 diabetes
  • Diabetes in pregnancy

This service requires a referral from your GP or Specialist and is available at Helensvale Community Health Centre and Robina Health Precinct.

Heart Failure Service

About this service

Heart failure is a chronic condition where the heart can be weakened and therefore may not fill or pump blood effectively to the body.

There are many reasons a person might have heart failure.

Symptoms include shortness of breath, fatigue, swollen legs and rapid heartbeat.

The heart failure team works with you and your family to learn how to care for yourself and manage your heart condition.

We offer specialist assessment, education, supervised exercise programs and a community-based heart failure clinic with a Cardiology doctor.

Can the service help me?

Yes, if you have:

  • A diagnosis of chronic heart failure confirmed by your GP or a Cardiologist

This service requires a referral from your GP or your Cardiologist and is available at Helensvale Community Health Centre and Robina Health Precinct.

Respiratory Service

About this service

A chronic lung condition and breathlessness can affect your independence, ability to perform basic day-to-day tasks and your quality of life.

The respiratory service will help you (and your family or friend) understand your condition, and support you as you learn ways to manage your symptoms.

The Respiratory Program is designed to help you manage your shortness of breath and lung condition. 

This service includes Pulmonary Rehabilitation which consists of education and supervised exercise programs.

You will be assessed by the respiratory team so that they can design a program to suit you.

Can the service help me?

Yes, if you have:

  • Chronic Obstructive Pulmonary Disease (COPD)
    • Emphysema, Chronic Bronchitis, Chronic Asthma
  • Bronchiectasis
  • Interstitial Lung Disease
    • Pulmonary Fibrosis, Silicosis, Asbestosis

This service requires a referral from your GP or Respiratory Physician and is available at Helensvale Community Health Centre and Robina Health Precinct.

Cardiac Rehabilitation Program

About this service

Many people who discover they have a cardiovascular condition were not aware that they were at risk. After a cardiac event, people recover better with support to achieve personal goals and protect their heart in the future.

The cardiac team offers education and exercise programs to assist your recovery. With support from the cardiac rehabilitation team, our program will help you modify lifetime habits that may be impacting your cardiac health.

Your program will be individually designed and supervised in a safe surrounding. Our specialist team works with you to help you enjoy an active and productive life.

Can the program help me?

Yes, if you have:

  • experienced a heart attack
  • had an angioplasty or open-heart surgery
  • a diagnosed cardiovascular condition
  • risk factors for heart disease

This service requires a referral from your Cardiologist or your GP and is available at Robina Health Precinct.

Vascular Rehabilitation Service

About this service

This exercise program aims to improve the quality of life, walking distance, and pain experienced by people living with peripheral vascular disease.

Your exercise program will be individually designed and supervised by a specialist team.

Can the service help me?

Yes, if you have:

  • A diagnosis of peripheral vascular disease confirmed by your GP or a Vascular Specialist

This service requires a referral from your Vascular Specialist and is available at Robina Health Precinct.

Falls and Balance Service

About this service

There are many reasons why someone may fall or be unsteady.

The Falls and Balance Clinic helps people identify why they are having trouble with their balance and/or falling over.

Our team includes a Doctor, Nurse, Pharmacist and Physiotherapist who are specially trained in conditions that affect your balance.

Once the team has determined your individual problem, they can recommend ways to help you improve and refer you to the right service. 

Can the service help me?

Yes, if you are:

  • Falling over
  • Unsteady or dizzy and you don't know why

This service requires a referral from your GP and is available at Robina Health Precinct.

Cognitive Disorders

About this service

There are many reasons why a person may have problems with memory. 

The Cognitive Disorders Clinic supports your GP by diagnosing the reason for a person's change in memory or thinking process (cognitive ability).

Can the service help me?

Yes, if you are:

  • Over 45 years old
  • Experiencing memory or cognitive changes
  • Do not already have a diagnosis, or where specialist input is required to support future plans for care

This service requires a referral from your GP and is available at Robina Health Precinct.

Mungulli: Aboriginal and Torres Strait Islanders Service

About this service

Are you of Aboriginal and/or Torres Strait Islander Origin? Mungulli is specifically designed for your community to access education and support around chronic disease.

The Mungulli Wellness Clinic is a culturally safe program that is designed to help you manage your health conditions through our specialised Chronic Disease team.

The Heart Health Screening Clinic is a culturally safe environment for the early detection and treatment of heart disease.

Both Mungulli and the Heart Health Screening Clinic help enhance healthy lifestyles for Aboriginal and Torres Strait Islander individuals, families and communities.

Can the service help me?

If you have:

  • Diabetes
  • Chronic Chest Problems
    • including Chronic Obstructive Pulmonary Disease (COPD) or Emphysema
  • Asthma
  • Chronic Kidney Problems
  • Coronary Heart Disease
  • Heart Failure
