Who we are 

Young Adult Continuing Care Service is a community-based specialist service that provides developmentally tailored and culturally considered care for young people between 18 to 21 years of age who are experiencing severe and complex mental health difficulties. 

What we do 

We provide a continuing care service that is personalised, accessible and youth friendly. We also provide evidence-based interventions in partnership with young people and their families. 

Who can access this service 

Young people between 18 to 21 years of age affected by serious and persistent mental illness (suspected or diagnosed) with significant impairment in functioning.  

How to access this service 

Referrals are accepted for young people between 18 to 21 years of age who meet the eligibility criteria from internal mental health services.

CYMHS community teams providing Gold Coast–wide care will continue to operate across multiple locations, including Proxima, the Robina Health Precinct and the Head to Health Kids site in Nerang, depending on what best meets the needs of the child or young person.
