About Transition Care
The Transition Care Program (TCP) provides time limited, goal orientated and therapy focused intervention to older people who require more time to complete their restorative process and optimise their functional capacity following hospitalisation.
The Program aims to support older people with the potential to benefit from TCP with individually tailored therapy, nursing care, and activity of daily living assistance to enable older people to further improve or maintain their physical, cognitive and psycho-social functioning.

Eligibility
Eligibility is determined by assessment abiding by the Aged Care Act (2024) and National Transition Care Guidelines. A current ACA approval is required for an older person to be admitted to TCP directly from hospital inpatient or the Hospital in the Home program.
Referrals
TCP accepts referrals from all public and private hospitals. Please call the TCP to discuss the referral process.
