The Placement Support Team is a multidisciplinary group comprising:
- Three Social Workers
- One Social Worker’s Assistant
- One Dedicated Administration Officer

Team members are located across both Gold Coast University Hospital and Robina Hospital.
When to refer patients to the Placement Support Team?
- Complex behaviours such as physical aggression, exit seeking behaviours, impulsivity or high risk of falls.
- Complex clinical needs e.g. PEG feeds, dialysis, bariatric patients, complex mental health issues.
- Cases involving the Office of the Public Guardian (OPG) and/or Public Trustee Queensland (PTQ).
- Placements for those aged under 65 when NDIS cannot support.
- Placements for First Nations People aged 50 years and above.
- Previous failed discharges to Residential Aged Care Homes due to responsive behaviours.
- Mental health diagnosis making the placement complex or impacting placement options
i.e. Electroconvulsive Therapy (ECT) or Ketamine treatments.
- Out of area placements i.e. interstate / out of Gold Coast catchment or patients from overseas wanting to go back to their home country.
- Complex finances e.g. family financial abuse.
- Patients with low care needs and are concessional.
- Patients with legal complexities including Domestic and Family Violence Order or identified of alleged victim of Elder Abuse.
To refer a patient to the Placement Support Team, please follow the steps below:
- Complete an Integrated Care Services Referral, ensuring you select “Placement Support Team” as the designated service.
- Email the completed referral to TSReferrals@health.qld.gov.au.
- Include detailed reasons for the referral to assist with triage and appropriate support planning.
Once received, the Placement Support Team will triage the referral and advise on the outcome.
