The Chronic Disease team includes Mungulli Clinics, heart health screening clinics, yarning circles and education.
This Aboriginal and Torres Strait Islander specific chronic disease management clinic offers support from an Indigenous health worker to access specialist nursing and allied health professionals while working closely with your GP.
We offer programs at a range of locations across the Gold Coast.

Better Cardiac Care Program
- Assisting patients with heart specialist appointments and procedures
- Be supported in all parts of your health care journey, from hospital stay to follow up in the community
Strong and Deadly Wellness Program
- 6-week program including education, cultural and physical activities
- Community-based client-focused program to inspire you to be the healthiest version of you
Mungulli Wellness Clinic
- Concerned about symptoms or have a chronic condition? Together we can come up with a plan that focuses on the health matters that are most important to you
Mungulli Exercise Class
- Weekly classes with an individual exercise program based on your needs
Yarn and Walk Program
For Aboriginal and Torres Strait Islander adults looking to improve self-management and stay active.
- Connect with your community in a friendly, safe space.
- Yarn about healthy lifestyles while walking and being active.
- Learn strategies to manage your health conditions and ongoing care.
- When & where: Mondays, 9:30am – 11:30am, Robina Cheltenham Oval, Cheltenham Park.
- Before you start: GP referral and a assessment with a Mungulli team member.
- Supported by Gold Coast Health and Gold Coast Primary Health Network.
Deadly Tracks Program
- Half-day Chronic Disease education program.
