Patients participating in the Remote Patient Care program are monitored Monday to Friday, during business hours. If you think you are experiencing a medical emergency: Call Triple Zero (000) immediately.
Healthcare is evolving, and so are the ways we care for patients. Remote Patient Care allows patients to receive support and monitoring from their care team while remaining in the comfort of their own home or community.
Using connected technology, care teams can securely monitor health information, communicate with patients, and respond when additional support or intervention may be needed. This helps patients stay connected without always needing to visit a hospital or clinic.
Remote Patient Care aims to:
This app makes it easier for you to stay engaged with your health through practical monitoring tasks, and provides your care team with a comprehensive dashboard to monitor your journey.
If your care includes monitoring, we will provide devices such as a blood pressure monitor, pulse oximeter or scales. The devices are yours to keep, so you can continue monitoring your health after completing the program.
Your smart monitoring device will connect with Miya Care, allowing your care team to monitor your health data.
You have the right to:
Your health information is handled with the same care and confidentiality as any other part of your medical record.
Your information is used to support your care - not for any other purpose.
