Who we are   

The Nurse Navigator Team provides support to individuals with complex mental health issues who frequently present to acute services at Gold Coast Health. 

What we do   

We focus on improving health literacy of individuals and supporting them to access relevant community and specialised services, with focus on prevention and early intervention. We aim to reduce fragmentation, mitigate barriers, educate, empower and coordinate individualised, patient centred care. 

Who can access this service?   

The team provides support to individuals who have presented to Gold Coast Health’s Emergency Department six times or more within a 12-month period. 

How to access this service 

Individuals are identified by the Nurse Navigator Team following frequent presentations to the Emergency Department or referred by the Emergency Department and Acute Mental Health teams. 

They must be aged 18 to 65 years old and from the Gold Coast region. 

The service operates from 8.30am to 5pm, Monday to Friday.
