Who we are   

The Nurse Navigator Team provides support to individuals with complex mental health issues who frequently present to acute services at Gold Coast Health. 

What we do   

We focus on improving health literacy of individuals and supporting them to access relevant community and specialised services, with focus on prevention and early intervention. We aim to reduce fragmentation, mitigate barriers, educate, empower, and to coordinate individualised, patient centred care. 

Who can access this service?   

The team provides support to individuals that have presented to Gold Coast Health’s emergency department six times or more within a 12-month period. 

How to access this service 

Individuals are identified by the Nurse Navigator following frequent presentations to the emergency department or referred by the emergency department and Acute Mental Health teams. 

Individuals must be aged between 18 to 65 years old, in the Gold Coast region. 

The service operates 8.30am to 5.00pm, Monday to Friday.
