Are you referring to the right service?

  • If the patient is experiencing post-concussive concerns, consider referral to Concussion Service
  • If the patient requires interventions to support the decision to reduce or cease substance use, consider referring to Alcohol and Other Drugs Service
  • If the patient requires short term rehabilitation within the community setting, consider referring to the Community Rehabilitation Program
  • If the patient is a paediatric and requires rehabilitation support, please consider referring to the Queensland Paediatric Rehabilitation Service
  • If the patient requires specialist outpatient consultation for acute brain injury, consider referring to the Specialist ABI Outpatients

Out of Scope Services

The following conditions are out of scope for this service:

  • Geriatric patients> 70 years age
  • Paediatric patients < 16 years of age
  • Medically unstable
  • Where primary mental health diagnosis or drug/alcohol dependency is the principal cause of issues impacting community integration.
  • Client is currently engaged in an intensive rehabilitation program.
  • Progressive neurological conditions
  • Neurodegenerative disorders
  • Congenital disorders
  • Mild traumatic brain injury or concussion

Out of Catchment

Gold Coast Health is responsible for providing public health services to the people who reside within its boundaries. Special consideration is made for patients requiring tertiary services or for services that are not provided by their local hospital and health service district. If your patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral.

  • Medicare card
  • Any concession cards (e.g. Pension, Health Care, DVA, PBS Safety Net, ADF, etc.)
  • Current medication list
  • Reading glasses, hearing and mobility aids
