Hospital avoidance pathway

Rapid in home clinical response from nursing and allied health professionals.

The Multidisciplinary Avoidance and Post-Acute Service (MAPS) team comprised of nursing and allied health professionals has been established to provide a hospital avoidance pathway for GPs and the Queensland Ambulance Service (QAS).

MAPS delivers a rapid in-home clinical response seven days a week and has the capacity to visit the referred person at home within 24 hours from the referral if required. The service provides assessment and practical bridging intervention for up to two weeks for individuals on the program after which, transfer of care to community care providers will be required for ongoing support. Medical governance of these patients will remain with the GP.
