Useful Management Information

  • The 0-5 Year clinics are an opportunity to have a consultation with an experienced Child and Family Health Nurse.
  • Consultations can support parents/ carers who have any specific concerns regarding their child’s health and development (non-urgent), it can also be for routine health and developmental checks.
  • The health and developmental checks are provided, as per the recommended schedule of; 0-4 weeks; 6-8 weeks; 4, 6, 12 and 18 months; 2, 3 and 4 years, as outlined in the Personal Child Health Record/ Redbook.
  • The checks involve routine tracking of a child’s growth and assessing developmental progress including gross and fine motor skills, speech, and language, and social and emotional development.
  • Health advice and guidance are provided with a focus on health promotion and building parent/carer confidence.
  • Anticipatory guidance can be provided, which includes nutritional information, feeding support, infant, and child behaviour.
  • During the appointment, the Nurses will also offer support and advice, where appropriate regarding parent/carer/ family health needs.
  • Referrals can be made to other specialist services if any additional support is required, such as Speech Pathology, Dietitian’s, Child Development services, non-government organisations and Early Years Centres.

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • No Category 1 criteria
Category 2 (appointment within 90 calendar days)
  • Children aged 0–5 years requiring timely health and developmental assessment where delay may risk missing early intervention opportunities.
Category 3 (appointment within 365 calendar days)
  • No Category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

  • Other stakeholder support (i.e. GP, NDIS, NGO, PNMH, Child Safety)
