Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)

Intervention within 3-5 working days

  • Dysphagia (swallow impairment) related to recent aspiration pneumonia;
  • Sudden deterioration in dysphagia/saliva management resulting in coughing/choking/aspiration;
  • Clients commencing thickened fluids on discharge;
  • Referrals marked as urgent by referrer

Intervention within 6-14 working days

  • New onset of dysphagia however still managing food/fluids/oral medications;
  • Ongoing management of swallow function/dysphagia if already assessed by another SP (i.e. post-acute);
  • Education and support of communication impairments (incl. dysarthria, dyspraxia of speech, cognitive communication disorders, dysphonia and aphasia) where functional communication is severely compromised, and client does not require rehabilitation

Intervention within 15-28 working days

  • Need for alternative and augmentative communication (AAC) for functional communication for progressive/ chronic conditions
  • Education and support of communication impairments where communication remains functional for daily needs and client does not require rehabilitation
Category 2 (appointment within 90 calendar days)

No category 2 criteria

Category 3 (appointment within 365 calendar days)

No category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

  • Does the client have any cognitive decline or capacity issues?If yes, has the patient received a definite diagnosis?

Private Health documents including:

  • Private Hospital Discharge summaries
  • Neurology reports (Last 3 years)
  • Geriatrician reports (Last 3 years)
  • Ear Nose and Throat (ENT) reports (Last 3 years)
  • Radiology imaging including Chest x-rays, Brain images, Barium swallow, modified barium swallow or videofluoroscopy swallow study
  • Medication summary
  • Speech Pathology reports and recommendations
