Useful Management Information
MAPS is a multi-disciplinary service (consisting of: Social Workers, Occupational Therapists, Physiotherapist, Allied Health Clinical Assistant, Nurses, Service Navigators) that provides supported discharge of patients that are medically fit for discharge, including rapid, practical bridging intervention until the commencement of usual services. In addition, MAPS provides Queensland Ambulance Service (QAS) with an avenue to support hospital avoidance for patients who are able to stay at home with short-term multi-disciplinary support. The service will operate seven days-a-week and will respond with a home visit to the patient within 24 hours of referral.
MAPS can provide short-term assistance with:
- Medication supervision
- Organising equipment/medical aids
- Assessing your home to ensure you are safe.
- Simple wound care
- Shopping
- Filling scripts
- Personal hygiene
- Transport where allied health intervention is required

Essential Referral Information
- All referrals need to be discussed with the MAPS team leader, please call 1300 00 4242 (1)
- Patient/decision maker agreeable to referral
- Patient has a nominated GP
- Other health providers involved in care have been notified
- Home Visiting Risk Assessment completed
