Useful Management Information
- Initial home visit to provide additional support postnatally up until 6-8 weeks of age.
- At the home visit the Child and Family Health Nurse will complete a 0-4 week or 6-8 week assessment, depending upon the infants age.
- Home support may be required where there are additional needs (i.e. feeding issues, prematurity, twin birth, no access to Medicare benefits).
- Consultations can support parents/ carers who have any specific concerns regarding their child’s health and development (non-urgent).
- Anticipatory guidance can be provided, which includes nutritional information, feeding support, infant, and child behaviour.
- During the appointment, the Nurses will also offer support and advice, where appropriate regarding parent/carer/ family health needs.
- Health advice and guidance are provided with a focus on health promotion and building parent/carer confidence.
- This service can be made by self-referral and other referrals from key stakeholders.
