Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)

Intervention within 3-5 calendar days 

  • Enteral Feeding – nasogastric or nasojejunal 
  • Nutrition Optimisation post-gastric surgery (that can’t wait more than five days to be seen) 
  • Severe Malnutrition (SGA-C) not seen as an inpatient 

 Intervention within 6-14 calendar days 

  • Bowel Disorders 
  • Eating Disorders 
  • Malnutrition (SGA-B, SGA-C) 
  • Post bariatric surgery 
  • Post Fundoplication Surgery 

 Intervention within 15 – 28 calendar days 

  • At risk of malnutrition (SGA-A) 
  • Autoimmune conditions 
  • Coeliac Disease 
  • Diverticular Disease 
  • GORD 
  • Hypercholesterolemia 
  • MAFLD 
  • IBS 
  • Type 2 Diabetes 
  • Weight Management including pre-surgery weight reduction 
Category 2 (appointment within 90 calendar days)

No category 2 criteria

Category 3 (appointment within 365 calendar days)

No category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Anthropometry (weight, height, BMI and recent weight changes) 
  • Medical and surgical history 
  • Social history 
  • Medication history 
  • Any recent pathology (more specific) 
  • List of allergies 
  • List of other providers currently providing care 

Additional Referral Information

  • If referring for pre-surgery weight reduction, please specify amount of weight required to be lost and date or proposed date of surgery
  • Please list any proposed upcoming appointments where the treating team dietary prescription may be changed including post-endoscopy and/or dilatations
