Useful Management Information

The biomechanical clinic provides Occupational Therapy services to children and young people who require specialised assessment and intervention for surgical or physical conditions affecting their daily occupations. Appropriate referrals may include; upper limb assessments (excluding those with a neurological cause), congenital hand abnormalities, scar management (eg burns scars or surgical scars not managed by GCUH Paediatric Surgery), arthritis affecting upper limb function, reconstructive surgeries and vascular malformations. The clinic is available weekly on a Wednesday morning from 8-11am. Both face to face and telehealth options are available. It is an OT only clinic but referrals to other MDT members can be made as appropriate. For external referrals, medical governance must be maintained at your service, or a re-referral pathway available for the GCUH OT as needed.

Patients needing frequent occupational therapy reviews are not suitable for this clinic, as it operates only one morning per week. Please consider making referrals after the acute phase of intervention.

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Requires intervention (Eg splinting) for prevention of a deformity eg severe Juvenile Idiopathic Arthritis (JIA), at risk scarring crossing a joint, congenital hand conditions.
  • History of problematic scarring
  • Identified risk factors eg skin breakdown, dark skin tone, infection
  • Facial scarring
  • Nerve associated conditions or injuries
  • Transfer of accepted vascular referrals from external service for timely garment fitting.
Category 2 (appointment within 90 calendar days)
  • Functional assessment for congenital hand condition that are not at risk of deformity without urgent care.
  • Transfer of accepted referrals for scar management, who have started treatment and do not require frequent reviews.
  • Healed surgical scars without identified risk factors
Category 3 (appointment within 365 calendar days)
  • No Category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Date of injury / onset of symptoms
  • Mechanism of injury
  • Location
  • Surgical details if relevant eg skin graft, muscle flap, wound closure
  • Length of time to heal for burns or wounds requiring scar management.

Additional Referral Information

  • Functional impact, including limitations related to pain and range of motion
  • Current medical management if relevant eg concurrent medications to mange symptoms
  • Summary of OT interventions to date
  • Psychosocial considerations eg procedural anxiety, hospital trauma etc
  • Identified risk factors eg history of poor scar outcomes, delayed healing due to infection or wound breakdown, dark skin tone, crossing a joint)
  • Indicate if shared care arrangement or transfer of care to GCUH
