Useful Management Information

  • Enhanced home visiting services are available for families who need extra support, due to complexities they may be facing which could impact on parenting, such as parent/carer mental health, substance misuse, domestic family violence, and social isolation.
  • Home visiting services are available by referral when a family may require additional ongoing support via either a ‘Sustained’ or ‘Coordinated Care’ home visiting program.
  • Home visits can be offered at regular intervals from the late antenatal period, up until the child reaches 2 years of age, depending on the need.
  • Child health and developmental checks will be offered as part of the program, along with parenting support and referrals to other services where indicated.
  • Advice and guidance on parenting issues can be offered through targeted interventions, to support parent/ carer confidence and to improve outcomes for their child/ children.
  • The Child and Family Health Nurse will provide a range of anticipatory guidance and advice to support the health and development of the child and improve family health outcomes; the Nurse can also refer to other support services as required.
  • The program can be offered in a clinic/community setting and/or virtually, depending on the family's needs.

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Families with infants or young children (from late antenatal period up to 2 years of age) who requireurgent enhanced home visiting support due to complex risk factors that may significantly affect parenting capacity, child health, or developmental outcomes, including:
  • Parental/carer vulnerabilities: mental health concerns, substance misuse, domestic and family violence, or severe social isolation.
  • High risk to child development/heFamilies with infants or young children (from late antenatal period up to 2 years of age) who requireurgent enhanced home visiting support due to complex risk factors that may significantly affect parenting capacity, child health, or developmental outcomes, including:
  • Parental/carer vulnerabilities: mental health concerns, substance misuse, domestic and family violence, or severe social isolation.
  • High risk to child development/health: where delayed intervention may adversely affect physical, emotional, or social wellbeing.
  • Need for intensive, sustained, or coordinated care: requiring regular home visiting and multidisciplinary support.
  • Limited capacity to access standard services: families unable to effectively engage with routine clinic-based care without additional outreach.

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service
