Useful Management Information

IMPRINT is a community-based multidisciplinary team which aims to reduce hospital length of stay through navigation and intervention of Gold Coast Health residents who have recently been approved a high-level Home Care Package (level 3 or 4 HCP) by the Aged Care Assessment Team (ACAT).

The team includes medical input, occupational therapy, nursing, social work and neuropsychology for assessment and short-term navigation.

Consider Chronic Disease Management Plan eligibility. Consider recommending patient/carers seek independent financial advice, advice via Centrelink

Please note that where appropriate and where available, the referral may be streamed to an associated public allied health and/or nursing service. Access to some specific services may include initial assessment and management by associated allied health and/or nursing, which may either facilitate or negate the need to see public medical specialist.

A change in patient circumstance (such as condition deteriorating or becoming pregnant) may affect the urgency categorisation and should be communicated as soon as possible.

Patient eligibility criteria:

  • Approved for a high-level (level 5-8) Support at Home Package
  • Gold Coast Hospital and Health Service patient or resident
  • Has a nominated GP
  • Requires an in-home review by a multidisciplinary team

IMPRINT can assist with:

  • Comprehensive geriatric assessment
  • Linkage with appropriate community supports
  • Advanced care planning
  • Carer and patient education
  • Education regarding community services, residential aged care and My Aged Care
  • Review of home environment and need for assistive devices
  • Support with placement and accessing services

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Patients who have been approved a high-level Support at Home package (level 5 to 8) and are at risk of a prolonged hospital admission
Category 2 (appointment within 90 calendar days )
  • No category 2 criteria
Category 3 (appointment within 365 calendar days)
  • No category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

  • Patient/decision maker agreeable to referral
  • Patient has a nominated GP
