Are you referring to the right service?

  • IMPRINT is a community-based multidisciplinary team which aims to reduce hospital length of stay through navigation and intervention of Gold Coast Health residents who have recently been approved a high-level Home Care Package (HCP) by the Aged Care Assessment Team (ACAT) or are currently awaiting assessment with these expected high care needs before they are admitted to hospital.
  • The team includes medical input, occupational therapy, nursing, social work and neuropsychology for assessment and short-term navigation with the ability to facilitate onward referrals. Risk factors targeted include undiagnosed cognitive impairment, unrealistic expectations, poor health and financial literacy and inadequate community supports, for example.

Out of Scope Services

  • A patient already living in residential aged care
  • Patients with high-risk Home Visiting Risk Assessments
  • Patients requiring low level (1-2) Home Care Packages or CHSP services

Out of Catchment

Gold Coast Health is responsible for providing public health services to the people who reside within its boundaries. Special consideration is made for patients requiring tertiary services or for services that are not provided by their local hospital and health service district. If your patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral.

Patient Must Bring

  • Medicare card
  • Any concession cards (e.g. Pension, Health Care, DVA, PBS Safety Net, ADF, etc.)
  • Current medication list
  • Reading glasses, hearing and mobility aids
