The Connected Care Program is a statewide network of local care coordinators that supports the needs of families who have children or adolescents with complex health care needs aged 0-16 years who see multiple specialists on a regular basis.

The program will:

  • Identify and create partnerships with members of the child’s health care team- including their GP, community and local care providers, hospital and healthcare providers and non-government organisations.
  • Act as a central point of contact for families.
  • Assist families to coordinate services required for their child.
  • Assist with appointment coordination where able.
  • Support, guide and empower families to manage their child’s health care needs
  • Ensure families understand their child’s health care needs, so they can make informed health decisions.
