Are you referring to the right service?

Eligibility criteria for the Concussion Service includes:

  • Confirmed or suspected diagnosis of concussion according to the American Congress of Rehabilitation Medicine definition within the last 12 months Individuals who have a history of one or multiple prior concussions, and a new concussion are eligible
  • Present with persisting concussive symptoms (with high symptom burden) for greater than 4 weeks and is impacting function, including activities of daily living, usual valued roles and/or vocation (work, study)
    • Earlier referral (i.e., before 4 weeks) may be appropriate for patients who have high symptom burden, are unable to return to usual activity / roles, and who have known risk factors for poor recovery
    • These referrals will be accepted where indicated
  • Individual is willing to actively participate in a goal-focused rehabilitation program
  • Aged older than 16 years
  • Reside in the Gold Coast Health catchment
  • Medicare eligible or eligible to receive government funded assistance

Out of Scope Services

Individuals who have sustained a concussion, but their injury was:

  • (1) greater than 12 months ago;
  • (2) have a history of moderate to severe Acquired Brain Injury (ABI); or
  • (3) demonstrate acute changes on brain injury (i.e., a complicated mild Traumatic Brain Injury (TBI))
    • These referrals would be diverted to the Gold Coast University Hospital Specialised ABI Outpatient Service.
  • Referrals for which there are factors that will significantly impact participation with this service. Including but not limited to, significant and unmanaged chronic health conditions or substance abuse.
  • Active compensation claim related to the injury.

Out of Catchment

Gold Coast Health is responsible for providing public health services to the people who reside within its boundaries. Special consideration is made for patients requiring tertiary services or for services that are not provided by their local hospital and health service district. If your patient lives outside the Gold Coast Health catchment area and you wish to refer them to one of our services, please indicate relevant medical or social information that will assist with the processing of your referral.

Patient Must Bring

  • Medicare card
  • Any concession cards (e.g. Pension, Health Care, DVA, PBS Safety Net, ADF, etc.)
  • Current medication list
  • Reading glasses, hearing and mobility aids
