Up to date information for your practice and GPs is required to ensure we can share patient charts and support correspondence electronically.

If your practice has any changes, please complete and submit the attached forms:

• Medical Practice STS update form
• Individual GP STS update form

When updating details, you may need to consider the following address books too:
