A urology specialist outpatient discharge and re-entry care pathway for patients after prostatectomy and nephrectomy has been established.

These patients require ongoing surveillance and care for many years after surgery which is best managed by their GP.

This service will facilitate timely communication between specialists and GPs and support GPs to provide ongoing care with a pathway for timely advice or re-entry to specialist care.

Should one of your patients be suitable for this pathway, a letter containing the follow up requirements and contact details for the service will be sent via Medical Objects.
