Smart Referrals is now the preferred method of making a referral.

The longstanding referral templates for Gold Coast Health Outpatients and Community Services are coming to the end of their lifespan.

All GPs using Best Practice or Medical Director are encouraged to switch to the Smart Referrals platform as soon as possible.

Until you transition to Smart Referrals GPs may continue using the old templates however you may find that more referrals are declined, and additional information is requested.

While using this old template only complete the fields as prompted. Please do not add additional commentary outside those fields or add images, photographs or other documents that are not prompted for in the template. Additional clinical information can be faxed to (07) 5687 4497.

For GPs using other clinical software please continue your current referral processes and we will advise when Smart Referrals becomes available.
