Our new Gold Coast Health Community based service IMPRINT (Integrated Multidisciplinary Proactive Risk Identification and Navigation Team) is now accepting referrals via Smart Referrals or your usual referral method.


IMPRINT aims to support Gold Coast residents who

  • have recently been approved or awaiting a high-level Home Care Package by the Aged Care Assessment Team (ACAT) and
  • would benefit from short term intervention in their own home by our multidisciplinary team including Geriatrician, GP with special interest, Occupational Therapist, Clinical Nurse Consultant, Social Worker and in the near future neuropsychologist.

IMPRINT will be holding a community consultation workshop in early 2024.
