The following template must be downloaded - please do not open the file without following the instructions below:

To install new templates:

1. Open Zedmed > Clinical Records > Tools > Clinical WP Setup > WP Templates

2. File menu > select Import – locate downloaded template and click Open

3. Type in template name into the Description field and click OK

To send a referral:

4. Make sure you're familiar with the latest criteria when referring your patient by using our Refer Your Patient website . You can search the relevant condition to find its Minimum Referral Criteria and Essential Referral Information. You must supply the essential referral information otherwise your referral will be returned.

We recommend adding our Refer Your Patient website  to your Favourites so that it appears as a a shortcut in your browser.

5. Open your patient file and use the new referral template installed following steps 1-3 above.

6. Follow your practice software instructions to send your referral.

Test your first referral:

Please email GCGPLU@health.qld.gov.au  with the patient’s name, DOB and the date you sent your first referral using the new template and we will confirm it has been successfully transmitted.
