Useful Management Information

  • Patient should not be referred to wound management clinic if there is a concurrent referral with another subspecialty for the same problem
  • Please outline if patient requires additional assistance with mobility or accessibility
  • Please identify any barriers to ongoing treatment of the patient

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Failure to adequately respond to maximal medical management, especially in the immunocompromised.
Category 2 (appointment within 90 calendar days)
  • No category 2 criteria
Category 3 (appointment within 365 calendar days)
  • No category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Co-morbidities and past medical history
  • History of injury including significant water, animal involvement or environmental exposure
  • Details of all treatments offered, and efficacy to date e.g. type of dressings used, date of commencement of any antibiotics with dose prescribed

Additional Referral Information

  • History of allergies and list of current medications
  • Relevant pathology (as clinically indicated)
  • Relevant medical imaging results if available –e.g. x-ray, ultrasound
  • Clinical photograph – with patient’s consent, where secure image transfer, identification and storage is possible
