Useful Management Information

Reactive thrombocytosis can be seen in:

  • Iron deficiency
  • Post splenectomy
  • Chronic kidney disorders
  • Acute blood loss
  • Haemolytic anaemia
  • Allergic reactions
  • Inflammation
  • Acute physical stress (MI, major surgery, burns, pancreatitis etc)
  • Drug side effects
  • Persistently elevated (over 3 months) platelet counts above 450 X 10/l that are not associated with iron deficiency or post splenectomy should be referred to Haematology.

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)

Persistent platelet level > 600 X 10/l:

  • Without iron deficiency or post splenectomy, and
  • Recent thrombosis or symptoms of hyper viscosity

  • Persistent platelet level > 1000 X 10/l regardless of symptoms
Category 2 (appointment within 90 calendar days)

Persistent platelet level > 600 X 10/l:

  • Without iron deficiency or post splenectomy
  • Asymptomatic
Category 3 (appointment within 365 calendar days)
  • Persistent platelet level 450- 600 X 10/l without iron deficiency or post splenectomy

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Serial FBC show monitoring over 3 months
  • Iron studies
  • CRP/ESR

Additional Referral Information

  • JAK2 V617F testing
  • Calreticulin mutation* (only if JAK2 V617F negative)
  • C-MPL* (only if JAK2 and Calreticulin negative)

- Can define essential thrombocythaemia

* Note: there is no Medicare rebate for these tests
