Useful Management Information

  • The Gold Coast Supportive and Specialist Palliative Care Service is a consultation service, working in conjunction with the patient’s GP, other clinicians and domiciliary nursing services.
  • The Community Palliative Care team provides:
    • outpatient or telehealth consultation services (home visits can be arranged if needed)
    • recommendations/suggestions to the patient’s GP and domiciliary nursing service following a review
    • referrals to the specialist allied health services for consultation
    • phone advice to GPs and other clinicians

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • The patient has a progressive or life limiting disease (malignant and/or non-malignant) and any of the following:
    • Symptoms that require specialist assessment or management, and which are beyond the capacity of the primary care team to manage
    • The patient and/or their family or carer have psychological, social or spiritual needs that require specialist assessment
    • The patient is dying, and the primary care team requires additional support and/or advice.

(Note: patients who are receiving life prolonging treatment are not excluded from palliative care)

Category 2 (appointment within 90 calendar days)
  • No category 2 criteria
Category 3 (appointment within 365 calendar days)
  • No category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Patient/decision maker agreeable to palliative care referral
  • Provisional diagnosis (cancer or other life limiting illness)
  • Symptoms and date of onset
  • Care at end of life requirements (if applicable)
  • Estimated prognosis
  • Other health providers involved in care
  • Does the patient have:
    • Advance Health Directive
    • Statement of Choices
    • Enduring power of attorney
    • Ability of GP to provide home visits or teleconferences

Additional Referral Information

  • Relevant medical history
  • Clinical examination findings
  • Treatment to date
  • Social factors and impact on patient (eg., activities of daily living, ability to care for others, personal safety/frailty).
  • Alcohol status/history
  • Smoking status/history
  • Medical history
  • Comorbidities
  • Previous procedures
  • Allergies / adverse reactions
  • Current prescribed medications
