Paediatric Surgery

Useful Management Information

  • Infection requires treatment with oral and topical antibiotics
  • Hygiene advice for retained keratin build up
  • Pathological phimosis
    • consider topical creams e.g. ½ strength betnovate for 4 – 6 weeks twice per day to inner preputial edge
  • A true diagnosis of DSD will result in Paediatric surgeon involving multidisciplinary team of paediatric medical specialists
  • In the majority of cases it is thought inappropriate for children to wait more than 6 months for an outpatient initial appointment
  • Next of kin or person(s) who is legally responsible for patient consent, with the exception of children under guardianship orders with the Department of Communities, Child Safety and Disability services, should be present at the first outpatient appointment
  • Paediatric surgery registrars at Queensland Children's Hospital/ GCUH can offer telephone advice to rural HHS. In some areas it would be more appropriate to seek initial advice from local paediatric medical service or general surgery services

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Episodes of acute urinary retention associated with poor urinary stream
  • New hypospadias preferably over 3 months of age (exclude cases that maybe intersex condition prior to referral to outpatients)
Category 2 (appointment within 90 calendar days)
  • Meatal stenosis not associated with retention
  • Pin hole preputial orifice with very poor urinary stream
  • Difficulty with retraction associated with ballooning or discomfort and failed trial of management by steroid cream
Category 3 (appointment within 365 calendar days)
  • Recurrent infective balanitis
  • Pathological phimosis
  • inability to retract foreskin after 7 years of age
  • Clinical indication for circumcision and failed conservative treatment
  • Difficulty with retraction associated with ballooning or discomfort and no other previous management

Essential Referral Information

  • Evaluate adequacy of urinary stream
  • History of any change in continence

Additional Referral Information

  • Hypospadias- aim for surgical repair at 9-12 months of age
  • Any family history of hypospadia
