Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Deep Venous Thrombosis (DVT)
  • Symptomatic Superficial Venous Thrombosis (SVT)
  • Venous ulcer healing
  • High risk of venous ulcer or reoccurrence
  • Vein procedure requiring compression stockings specifically in GCH facility
Category 2 (appointment within 90 calendar days)
  • Significantly symptomatic venous disease
  • Significantly symptomatic post cellulitis
  • Significantly symptomatic post-thrombotic syndrome (PTS)
  • Vein procedure requiring compression stockings specifically in GCH facility
Category 3 (appointment within 365 calendar days)
  • Mildly symptomatic venous disease
  • Mildly symptomatic post cellulitis
  • Mildly symptomatic post-thrombotic syndrome (PTS)
  • Vein procedure requiring compression stockings specifically in GCH facility

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Clear diagnosis of venous referral condition with relevant details
  • Category for vein procedure being completed in GCH facility
  • Clear wound management plan in place (if wounds present) and suitability for compression stockings
  • Limb/s requiring assessment for compression garments
  • Class of compression requested or if seeking advice of therapist
  • Any previous medical history or current medical issues that may impact compression garment tolerance (eg. Cardiac/kidney/liver function, arterial insufficiency, peripheral neuropathy, diabetes, HTN control or skin allergies)
  • Clearing stating medical clearance being provided for compression garments
  • Assessment for the presence of pedal pulses noted. Completion of Ankle Brachial Pressure Index (ABPI) may be requested by treating therapist if needed to support clinical assessment for suitability for compression garments
