Minimum Referral Criteria
Category 1 (appointment within 30 calendar days) | For optimum care, these conditions should be seen within 2 working days from acceptance of referral:

Essential Referral Information
- Reason for referral (list the condition/s requiring dietetic support during pregnancy)
- Stage of pregnancy or Estimated Date of Delivery (EDD)
- If referring for Diabetes in pregnancy or GDM, please provide at least 3 post prandial BGL
