Useful Management Information

The FeNO test can detect eosinophilic airway inflammation, determine the likelihood of corticosteroid responsiveness, monitor airway inflammation to determine the need for corticosteroids and identify non-adherence to corticosteroid therapy.

Indications:

  • Assess the etiology of respiratory symptoms.
  • Help identify the eosinophilic asthma phenotype.
  • Assess the potential response or failure-to-respond to anti-inflammatory agents, notably inhaled corticosteroids.
  • Establish a baseline FeNO during clinical stability for subsequent monitoring of chronic persistent asthma.
  • Guide changes in doses of anti-inflammatory medications step-down dosing, step-up dosing or discontinuation of anti-inflammatory medications.
  • Evaluate adherence to anti-inflammatory medications.
  • Assess whether airway inflammation is contributing to poor asthma control, particularly in the presence of other contributors (e.g. rhinosinusitis, anxiety, gastro-esophageal reflux, obesity and continued allergen exposure).

Clinical advice:

  • Cigarette smoking suppresses FeNO which limits its use in smokers.

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category Category 1 ((appointment within 30 calendar days))
  • Requires tests within 30 days.
Category Category 2 ((appointment within 90 calendar days))
  • Requires tests within 90 calendar days.
Category Category 3 ((appointment within 365 calendar days))
  • Requires appointment within 365 days.

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Reason for referral - what is the clinical question to be answered by performing the test?
  • Current respiratory medications
  • Infectious status

If a specific test result is unable to be obtained due to access, financial, religious, cultural or consent reasons a Clinical Override may be requested. This reason must be clearly articulated in the body of the referral.
