Useful Management Information

  • Adequate chronic disease/lifestyle (SNAP ) management is a requirement for most surgical procedures
  • Active smokers may not be considered suitable for a range of surgical procedures

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Fracture ulna/radius/humerus not requiring emergency department referral
Category 2 (appointment within 90 calendar days)
  • Suspected peripheral nerve dysfunction with associated motor deficit
Category 3 (appointment within 365 calendar days)
  • Other elbow conditions causing persistent pain or functional impairment despite attempts at non-operative management including physiotherapy/occupational therapy and simple analgesics

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Pain history
  • Functional impact including on employment if applicable
  • Management to date
  • Plain Xray
  • Ultrasound if Xray normal
  • MRI not required but negates other imaging if obtained

Additional Referral Information

  • Previous surgery
  • Electrodiagnostic studies if peripheral nerve dysfunction suspected
