  • Post acute cardiac event and/or post cardiac surgery
  • Cardiac Rehabilitation operates 5 days per week from 0700 – 1600 Monday to Friday
  • The Cardiac Rehabilitation delivers an exercise and education service to post-acute cardiac events and/or post-surgery patients and high-risk clients requiring risk factor and lifestyle modification education
  • The exercise program consists of 4-6 weeks of supervised exercise and offers a multi-disciplinary approach in improving survival post after an acute cardiovascular event

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category Category 1 (appointment within 30 calendar days )

Intervention provided within 6 - 14 calendar days

  • Hospital discharges following an acute cardiovascular event and/or surgery
  • One or more risk factors of cardiovascular disease: smoking, unhealthy weight, high cholesterol, high blood pressure, depression and social isolation, diabetes, physical inactivity, chronic stress
Category Category 2 (appointment within 90 calendar days)
  • No category 2 criteria
Category Category 3 (appointment within 365 calendar days)
  • No category 3 criteria

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service

Essential Referral Information

  • Confirmation of primary diagnosis of cardiology condition
  • Date and type or cardiac surgery
  • Current medication list
  • Pathology (most current or within the last 12 months):
    • FBC
    • Lipids
    • ELFTs
    • HbA1C
    • Troponin

Additional Referral Information

  • ECG
  • Coronary angiography
  • Myocardial perfusion scan
  • Echocardiography and/or exercise stress test results
