The Mungulli program delivers holistic chronic disease management through multidisciplinary team–led clinics, yarning circles, and health education. It is a culturally specific service for Aboriginal and Torres Strait Islander peoples, providing Indigenous Health Worker support to facilitate access to specialist nursing and allied health services in collaboration with GPs.
The multidisciplinary team (MDT) supports clients to understand and self-manage their health conditions by recognising symptoms and changes, building confidence to engage with health professionals, and developing individualised care plans and goals. Community education is also delivered across a range of conditions, including diabetes, chronic respiratory disease (e.g. COPD, emphysema, asthma), chronic kidney disease, cardiovascular disease, and social and emotional wellbeing.
Strong and Deadly Wellness Program
A community-based and client-focused 5-week program including chronic disease education, cultural and physical activities, facilitated in collaboration with Kalwun AMS at the Kalwun Wellness Centre in Bonogin.
Mungulli Wellness Clinic
A multidisciplinary Team approach, working with patients to create plans that focuses on the health issues that matter most to them. Clients have access to Physiotherapist, Exercise Physiologist, Pharmacist, Social Worker, Clinical Nurse, Diabetes Nurse Educators and Dieticians, supported by Mungulli Indigenous Health Workers.
Mungulli Exercise Class
Physiotherapist lead 12 week program, with individual exercise programs tailored for each specific client, supported by the Indigenous Health Worker and Clinical Nurses.
Deadly Tracks Program
A half-day Chronic Disease education program conducted by Chronic Disease Multidisciplinary team: Cardiac Nurse, Pharmacist, Diabetes Nurse Educator, Physiotherapist, A&TSI Nurse Navigators, Dietician.
Yarn and Walk
‘Yarn and Walk’ program is a group-based walking and education program that helps clients to build healthy habits, by providing simple tools to improve health and wellbeing.
Healthy lifestyle yarns client and their families to gain a better understanding of the importance of healthy eating, physical activity, and relaxation.
The program creates a culturally safe space for clients to build social connections. To attend the program, clients must have a nursing and physiotherapist assessment with the Mungulli team, be independently mobile without an aid and have good balance. Yarn and Walk is conducted weekly at the Cheltenham Oval Robina.
