Useful Management Information

The First Nations Dietetic Service provides individual and small group education to help Aboriginal and Torres Strait Islander patients manage or prevent diet-related conditions. Where appropriate, we work closely alongside medical practitioners to advocate for the role of nutrition in their patients’ wellbeing. We partner with Kalwun Aboriginal Medical Service to offer services in a variety of outreach locations within the Gold Coast Health Service catchment. The service operates within business hours Mon-Fri, excluding public holidays.

Minimum Referral Criteria

Does your patient meet the minimum referral criteria?
Category 1 (appointment within 30 calendar days)
  • Malnutrition Subjective Global Assessment A, B or C
  • Urgent post-acute follow up (e.g. hypermetabolic/ increased requirements, cancer care, kidney disease, immediate post-bariatric surgery, pancreatitis, end stage liver disease)
  • Antenatal: hyperemesis; high risk (iron deficiency, AOD, young age, multiparous, cat III obesity: pre-natal BMI>40)
  • Newly diagnosed food allergy
  • Ileostomy with high output
  • Diabetes: newly diagnosed (all types)/ Suboptimal control
  • Eating disorders
  • Weight reduction for urgent (<4wks) surgical intervention including pre-op Very Low Calorie Diet (VLCD)
Category 2 (appointment within 90 calendar days)
  • Micronutrient deficiencies
  • Impaired Glucose Tolerance (IGT) / PCOS
  • Cholecystitis/ Gallstones
  • Bowel disorders: IBD, coeliac disease, bowel obstruction, fibre modification, bowel surgery, ileostomy/ colostomy
  • Chronic GI symptoms (reflux/GORD, IBS, constipation, diverticular disease, food intolerance)
  • Lifestyle-related conditions (dyslipidaemia, MAFLD, kidney stones, gout, HTN)
Category 3 (appointment within 365 calendar days)
  • Lifestyle modification: healthy eating, general nutrition advice, chronic disease prevention, weight management
  • Annual diabetes review (all types, optimal control)

If your patient does not meet the minimum referral criteria

  • Assessment and management information may be found on a range of conditions at HealthPathways
  • If the patient does not meet the criteria for referral but the referring practitioner believes the patient requires specialist review, a clinical override may be requested:
    • Please explain why (e.g. warning signs or symptoms, clinical modifiers, uncertain about diagnosis, etc.)
  • Please note that your referral may not be accepted or may be redirected to another service
